Diabetes Management

Our Nurse Practitioner Dr Min Ding will coordinate your diabetes health care by working closely with your GP team and specialists as well as allied health professionals such as Exercise Physiologist and pharmacist to improve your diabetes management aiming to make you live better with higher quality of life with diabetes.  In order to be seen in diabetes clinic, you can either call the clinic to make an appointment for diabetes review if you are a patient diagnosed with diabetes or recently discharged from hospital with diabetes.  Or you can ask your GP Dr Yap or our practice nurse to refer you to our diabetes clinic.  

Your diabetes review journey will start from our practice nurse performing essential checks such as eye test, ECG and observations, then you will be seen by Dr Min Ding for a comprehensive diabetes review consult. Dr Min will work collaboratively with all other health professionals involved in your diabetes care, provides you with diabetes education and follow up appointments via either face to face or telephone consultations , she will also track your condition and alert treating doctors if your conditions change so early detection and treatment can be initiated for your diabetes. Through regular collaborative video conference with diabetes specialists, our multidisciplinary diabetes care is aiming to make you enjoy a more accessible diabetes care with higher quality.

Please remember to bring your medication list or relevant documents and any questions you may have on your diabetes on the day of your first appointment.
