Reminders

· Bring your Medicare card to appointments.

· Wear your hearing aid device when attending appointments.

· Bring Drivers Licence renewal paperwork, and Drivers Licence for a Drivers Licence Medicals.

· Bring your mobile phone to register for SMS reminders.

· A Pathology request form is required for all Pathology appointments. Please kindly drop the form to us one day prior if the form is issued by clinicians outside of our practice.

Welcome Dr Ryohei Tanaka

Let's give a warm welcome to our new GP registrar Dr Ryo Tanaka who will join our team starting from 3rd August. Dr Ryo is keen to look after rural people with Dr Jack in our practice. Please give him a warm welcome and understanding to help him settle in our friendly town and new practice.

Public Holiday WA Day clinic closure

Please note our practice will close on Monday 1st June WA Day Public Holiday and resume work on 2nd June. Please use our website or Health Engine for online booking. Our phone will not be answered on Monday. Enjoy the long weekend.
