Heart Health Clinic

Our Nurse Practitioner Dr Min Ding will coordinate your heart health care by working closely with our GP team and your cardiologist as well as allied health professionals such as Exercise Physiologist and pharmacist nearby to improve your heart function aiming to make you live better with higher quality of life with heart problems.

In order to be seen in heart health clinic, you can either call the clinic to make an appointment for heart health check if you are a patient diagnosed with  heart disease,  or recently discharged from hospital with heart disease or wishing to have a heart health assessment for prevention of heart disease. Or you can ask your GP Dr Yap or our practice nurse to refer you to our heart health clinic.

Your heart health care journey will start from our practice nurse performing essential checks such as ECG and observations, then you will be seen by Dr Min Ding for a comprehensive heart health assessment consult. Dr Min will work collaboratively with all other health professionals involved in your heart care, provides you with heart health education and follow up appointments via either face to face or telephone, she will also track your condition and alert treating doctors if your conditions change so early detection and treatment can be initiated for your heart health. Through regular collaborative video conference with experienced cardiologists, our multidisciplinary heart health care is aiming to make you enjoy a more accessible heart care with higher quality.
